This is a list of example prices for services that are not covered by health insurance at our clinic. Our prices follow the Ontario Medical Association’s suggested fees and are checked against local clinic rates to make sure they are fair. If you want, you can compare our prices with other nearby clinics that share their pricing online.

Prices may change from time to time. For the most accurate and up-to-date prices, please check in at the clinic. If you want to be sure before your visit, call us to confirm the current price.

We process payments with Square, which works with any Canadian-issued and most internationally-issued cards, and also supports mobile wallets such as Apple Pay, Google Pay and Samsung Pay. We also accept cash, although we do not carry any change.

Services

• Consult for patients without valid OHIP or Canadian provincial (Quebec excluded) health insurance
 – $100
 – $60 for a follow-up of the same issue within 2 weeks

A letter from the doctor will be provided for you to submit to your insurance company for consideration of reimbursement

• TB skin test (for purposes other than school, Ministry of Health requests, Long-Term Care admission and physician-deemed medically necessary purposes)
 – $110 uninsured (includes visit) for administration
 – $50 uninsured for reading
 – $30 per visit for insured

• Prior authorization for private insurance coverage of a medication (for Ozempic and Mounjaro please be advised that non-diabetics will typically have coverage rejected): $50

School and Workplace
• Proof of Immunization Forms: $75

Liquid Nitrogen
• Skin tag removal with liquid nitrogen - $40 per skin tag
• Wart treatment with liquid nitrogen (excluding genital and feet, which are OHIP covered) - $30 per session (will likely require multiple sessions)

Miscellaneous
• CAS – Medical Report (Adoption / Foster / Kinship / Customary Care Applicants in Ontario): $120
• Uninsured Prescriptions (Refills, Orthotics, Massage, etc.): $25 each if sole purpose of visit
• Prescription refill without a visit - $40 and at physician’s discretion. Must be a patient of the family practice. If you need an urgent refill it is preferred that you walk-in for a refill
• Records release to a patient or physician - charge is waved for digital copy
• Records release to another party - please contact clinic to review

No show to appointment
• Family practice visit - charge is waived
• Specialist consultation (Men’s Health, Metabolic Health) - at physician discretion
• Pap test with Dr. Vanderkooy - $50
