Do I need a referral for specialty services?

After your visit — results, referrals & forms

When will I get my test results?
Dr. Kamphuis checks results every day. Urgent results get a phone call; most others are emailed. We do not use a “no news is good news” rule — if you haven't heard from us after the times below, please email or call.

How do I check on a specialist referral?
Please allow two weeks for a referral to be processed before checking in. After that, if you haven't heard from the specialist's office, contact us and we can provide their phone number so you can follow up directly.
I need a form, letter, or sick note
All forms and sick notes require an in-person appointment. Book online, bring the form (or email it ahead to info@midtown.clinic), and pay at the visit — card, tap, or cash. Some forms take a week or longer. Prices are listed in the fee guide.
