Patient Enrolment and Consent
What is the CCM practice model?
By signing the MOH enrolment forms, your family doctor becomes your main contact when you need help with your health. They can check your symptoms, treat common problems, help prevent illness, and send you to specialists when needed.
Being part of this model means:
You can still see your own family doctor during normal office hours.
The group offers some extended hours, like evenings or weekends. These are available on the Calendar
You can call the Telephone Health Advisory Service (THAS) after hours. A nurse will give you advice and can contact a doctor from your group if needed.

Your doctor gets updated information about your health, which helps keep your medical record complete and accurate.
How to Join
To join, you simply sign the Patient Enrolment and Consent Form and give it to your doctor. By signing:
You agree to try to contact your family doctor or their group first when you need care (unless it is an emergency or you are away).
You allow the Ministry of Health to share basic details about other health services you receive outside the group, so your doctor can keep your record up to date.
Questions and Answers About Enrolment
Can I still see specialists or other health providers?
Yes. You will see your family doctor first, and they will refer you to specialists or other providers when needed.
Is my medical information kept private?
Yes. Your doctor and their team must protect your health information. They only share it with other health professionals when it is needed for your care. Any electronic sharing is done under strict privacy rules.
Can I go to the emergency department if needed?
Yes. If your child or family member is very sick, go to the emergency room right away. If you are unsure, you can call THAS after hours to get advice from a nurse.
What information am I agreeing to share?
By enrolling, you allow the Ministry of Health to give your doctor basic information about:
your immunizations
mammograms
cervical and colon cancer screening
other primary care visits you may have had at different clinics
You also allow THAS nurses to confirm who you are using your name, address, birth date, and Health Card Number. With your permission, they can send a note to your doctor after you call THAS.
More Questions About Enrolling
Can I register my children?
Yes.
You can enrol children under 16 by signing for them.
Teens 16 and older must sign for themselves.
Can I register someone if I have Power of Attorney for Personal Care?
Yes. Sign the form and check the “Attorney for Personal Care” box. The office may ask to see the document.
Can I change my doctor later?
Yes. You can switch to a new doctor up to two times per year. When you enrol with a new doctor, the old enrolment ends automatically.
Can I still get a second opinion?
Yes. Your doctor can help refer you, or you can choose another doctor for a second opinion.
What if I see a different family doctor regularly?
If you regularly see another family doctor, your enrolled doctor may remove you from their group roster.
