Getting Started as a New Patient

Are you accepting new patients? How can I register for family practice at Midtown Medical Center? Please see details here.

What happens after I fill out the patient form and share my health card? Give us a call or send us an email to book a meet and greet appointment with the doctor. That's your next step!

Do I need an appointment to become a new patient? Yes! Coming in for a meet and greet is how we get to know you. It also helps the clinic — we only get paid by the government when we actually see you as a patient. So meeting with the doctor is an important first step for both of us.

Can I still register if my health card is expired? If you have a temporary health card, that works just fine. If your card is fully expired, you will need to pay a private rate for your visit. Once you get your updated card, we can sort things out from there.

Appointments & Walk-Ins

Do you accept walk-ins, or do I need to book an appointment? Walk-ins are welcome, but appointments can also be booked ahead of time through our booking page. You do not need to be a registered family practice patient to book an appointment or to be seen as a walk-in.

How do I book an appointment? We prefer online booking, but appointments can also be made by phone or in person. You can also book online, email, or call the clinic for specific concerns like a recheck.

What if I am sick on the weekend or in the evenings? If you have a medical emergency, please call 911 or go to the emergency department.

If your condition can wait, please book or walk in on the next business day.

If it is non-emergent but cannot wait, you can visit an after-hours walk-in clinic in the region. One option is Onyx Urgent Care. As part of his funding agreement with the Ministry of Health, Dr. Kamphuis provides after-hours coverage at Onyx once per week for urgent medical problems.

If I'm worried about something, should I come in right away or wait? Please come in sooner rather than later. We would much rather see you too early than too late. Your health comes first!

How soon should I book after leaving the hospital — for example, after having a baby? The doctor at the hospital will let you know when to come in. Follow their advice on timing.

About the Clinic & Our Doctors

Where is Midtown Medical Center located? 702 King St W in Kitchener, Ontario, close to WRHN @ Midtown (formerly Grand River Hospital). We are at the doors to the right of the building.

Who will be my family physician? Dr. Andrew Kamphuis is the only family physician currently accepting new patients and seeing patients in both the family practice and walk-in clinic. Occasionally, a trusted colleague will cover for him. The clinic will only ever have licensed Canadian Medical Doctors (MD) seeing patients.

Where did Dr. Andrew Kamphuis train? Dr. Kamphuis completed his undergraduate studies in Health Sciences at McMaster University, followed by medical school at the same institution. He completed his family medicine residency at Queen's University, then pursued additional training in acute care medicine. He has been practising in Kitchener for four years.

Do you have a female doctor? Patients can access a female doctor at the clinic on a referral basis for pap testing. If you need to see an Obstetrician or Gynecologist, we would be happy to facilitate a referral. Please see details here.

What is TrueNorth Metabolic and how can I access this service? TrueNorth Metabolic is a service offered by Dr. Khashayar Farzam, providing focused weight-loss support and men's health care — though he sees patients of all genders. For further details, please click here.

Clinical Services

Do you offer specialist referrals or work notes? Yes! We are happy to help with referrals, work notes, and other paperwork through an in-person appointment.

How do I get a referral to a specialist or a fertility clinic? Come in for an in-person appointment to discuss it with the doctor. They will get the referral started for you.

Do I need a prescription to get a test like an MRI? Yes. Please book an appointment with the doctor first. They will decide what is needed and write the order for you.

How much does it cost for the doctor to fill out forms not covered by OHIP — like disability forms or insurance papers? Please check our fee guide for the most up-to-date prices. We are happy to help you find the right information.

How can I pay for forms or services not covered by OHIP? We can send you an online invoice through our payment platform, or you are welcome to pay in person at the clinic — whatever works best for you.

How do I send in records from my old doctor? Fax is the easiest way! We also accept USB drives and other formats if needed.

How do I get copies of my results or my child's growth charts? Just email or call the clinic and we will help you out.

My blood pressure was high at my appointment. What should I do? Don't worry too much yet! Many people have higher blood pressure in a clinic setting than they do elsewhere. Try checking it at home with an affordable cuff, or at a pharmacy or grocery store with a free machine. If it is still high, please come back to see the doctor.

Should I keep taking my medication — like iron pills — after something like a pregnancy? Yes — please keep taking all your medications until your doctor tells you to stop. Running out of medication does not mean it is time to stop. It means it is time to come in so the doctor can check how you are doing and renew your prescription.

Clinic Policies

Will I be delisted from the family practice for going to another clinic? No. In general, you will only be refused service for verbal abuse, aggression, or harassment toward staff or others.

Will I be delisted for using alternative health practices? No. The clinic and its practitioners follow evidence-based medicine but take a non-judgmental approach. In general, you will only be refused service for verbal abuse, aggression, or harassment toward staff or others.
